How the Evaluation Process Works

Getting started is simple. We guide you through each step of the Secretary of State substance use evaluation process from start to finish.

Step 1: Complete Our Contact Form

Start by completing the contact form on our website. Be sure to provide a valid email address and phone number so we can contact you regarding the next steps. After we review your information, we will send you an invitation to create a secure client portal account through Sessions Health.

Step 2: Create Your Secure Client Portal Account

Using the link we provide, create your confidential client portal account. This secure portal will be used for all communication, paperwork, document uploads, payment, and delivery of your completed evaluation.

Step 3: Complete Intake Forms & Upload Required Documents

Once logged into the portal, complete all required intake paperwork and upload the documents required for your evaluation. Required documents are listed in the Michigan Secretary of State SOS-257 instructions and may include:

Step 4: Notify Us You're Ready to Schedule

After completing your paperwork and uploading your documents, send us a secure message through the client portal or contact us directly to let us know you are ready to schedule. We will review your file and arrange a convenient virtual appointment time.

Step 5: Complete Your Virtual Evaluation

Attend your scheduled evaluation from the comfort of your home using our secure telehealth platform. During the appointment, your evaluator will review your history, discuss your recovery journey, and gather the information needed to complete the evaluation required by the Michigan Secretary of State. Payment is securely completed through the client portal.

Step 6: Receive Your Completed Evaluation

Your completed evaluation will be uploaded to your secure client portal within 7 business days of your appointment. You may then download the evaluation and submit it to your attorney or the Michigan Secretary of State as part of your driver's license restoration or clearance process.
